APPLICATION DIRECTIONS

There are 3 ways to submit your application. Please pick one and follow the simple instructions

Email Application- Open the “Application” form. Complete all required blocks. Either go into your options to send as an attachment or save in a document on your computer. Open email and send to  Pal@bensalem-township.org . Attach your saved document and send. You will be notified that we received your email.

YOU MUST ATTACH A COPY OF THE REQUIRED IDENTIFICATION. (See application)

Receiving your email will be an electronic confirmation that you agree to the terms of the liability form. If you don’t agree don’t send.
                                                                         
US MAIL Print and complete the entire application form and sign where required. Mail the application and a copy of the required identification to:

DROP OFF APPLICATION  Print and complete the entire application form, sign where required. Bring completed application and a valid identification to:

Police Dispatch window located at the police entrance.

2013 PAL BASKETBALL APPLICATIONS ARE NOW BEING ACCEPTED.
